a tournament, use this link to download a form that you
can fill out and fax such a request to us.

the pdf file byfollowing the download directions on the
right.

Windows
- Right click on the link and select Save file from the
pop up menu. Pick a destination location. Then open the
PDF file in Acrobat Reader.
Macintosh - Control - click on the link and
select Save link as, then select format as source. Pick
a destination location. Then open the PDF file.
